UPCPMT Application Form 2015 is now available. The application form of UPCPMT 2015 has been issued online at www.upcpmt2015.co.in. The UP Combined Pre Medical Test is being conducted for admission to MBBS, BDS, BHMS, BAMS, BUMS courses on May 24, 2015 by DDU Gorakhpur University. Before filling up the application form of UPCPMT 2015, candidate must check the eligibility criteria. Detailed instructions on how to fill the UPCPMT online application form 2015 are also given here. - Step 1 – Register online at www.upcpmt2015.co.in by entering name, gender, email ID , and mobile number. You will received log in credentials including password at the mobile number, and email.
- Step 2 – Enter details asked in the UPCPMT 2015 online application form such as date of birth, category, center choice, educational qualification. Upload photograph, signature, declaration form, right hand index finger impression.
- Step 3 – Pay application fee either online (via credit / debit card) or offline (via challan).

UPCPMT 2015 examination is being conducted by Deen Dayal Upadhyaya Gorakhpur University, Gorakhpur at various centres in Uttar Pradesh on May 24, 2015 (Sunday). Application Form of UPCPMT 2015 is now available on the website www.upcpmt2015.co.in from 20.03.2015 to 22.04.2015. The important dates you must know before proceeding to fill up the form are as follows.
- Last date to pay application fee by challan in the bank is April 20, 2015.
- Last date to submit the application form online and pay fees online is April 22, 2015.
UPCPMT Form Apply Online – Click here to fill the online application form of the Combined Pre Medical Test 2015. Last date to fill the form is April 22, 2015 (5 PM). You must have a valid mobile number, and email ID to register for the exam.
Declaration Form – Click here to download the declaration form. Give fingerprint impression of your right index finger in the given boxes without re-inking your finger. Ink your finger on the stamp pad once then apply it on the first box without smudging the impression, then apply the same finger on the second box without taking the ink again, repeat the same on the third box without re-inking your finger. For sample handwriting. Write the sentence given in the declaration form in your usual handwriting. This form must be attested by Principal or Gazetted officer with seal.

- The application form must be filled online only. Separate applications by post or any other means shall not be accepted.
- Candidate must fill in correct and verifiable details only in the UPCPMT application form 2015. Request for correction are not entertained.
- Incomplete application forms are liable to be rejected.
- For future reference, candidate must note down the application ID number.
After applying for the exam, you will need to download the admit card of UPCPMT 2015 as per schedule announced.
UPCPMT 2015 Online Application Form Filling Procedure
Candidate can fill the online application form of UPCPMT 2015 from any computer with internet access. Prior to filling up the form, it is strongly advised that the information brochure is read carefully. During filling up the form, you will require scanned photograph and scanned declaration form. Keep these ready before proceeding.
Registration and Log in – A first time user (applicant) will be required to register at the www.upcpmt2015.co.in website. To do this, he / she will primarily require a valid email ID and a mobile number.
Once registered, you will be issued a user ID, and password. This will be sent to your email ID, and mobile number. Use these login credentials to log in at the registration / login page on www.upcpmt2015.co.in website.
Filling up details in the application form of UPCPMT 2015 – Registered users can log in and enter all the details in the form. Personal information, educational qualification, communication details will need to be entered.
Upload the necessary documents as asked in the form as well.
Payment of application fees – Applicant has to click on the pay fees link and proceed as per instructions being displayed on the page. You can opt for either online payment, or offline payment. The application fees are as follows – Rs 1400 for General / OBC candidates, and Rs 700 for SC / ST candidates. This includes Rs 50 counselling fees. Bank transaction charges as applicable shall also be levied.

In the application form, generate bank challan of SBI.
Take the generated challan, and amount of Rs 1400 + 50 for General and OBC candidates, and Rs 700 + 50 for SC / ST can be deposited in the State Bank of India account number as given in the brochure, in cash. You will be given a financial transaction reference number / journal number / challan number. This number must be entered in the application form.
Instructions for Photograph
- The scanned photograph must be in .jpg format. Candidate must adhere to the instructions pertaining to the photograph to be uploaded in the UPCPMT Application Form 2015, else form may be rejected.
- The photograph must be colored.
- During getting the picture clicked for the application form, on a small placard, write your name, and date of taking the photograph. Place the placard in front of your neck, in such a way that it is visible in the photograph.
Keep 10 copies of this passport size photograph. These will be used in your form, counseling, and other times. Sample photo is given below.
From the website, you can download the declaration form for submitting with the application form of UPCPMT 2015. The information given in the form, and photograph must be attested by gazette officier or college principal. This document must then be scanned and uploaded. It shall 3.5 x 4.5 cms photograph, sample handwriting, right hand index finger impression, signature of guardian.

The home page screen will appear. Download information brochure UPCPMT-2015 from available link and read it carefully. Download course wise syllabus and read it carefully. Download Declaration form from available link, read and fill it according to given information and fill it according to information available in information brochure.
For new registration and re-login click on How to Apply link or Registration/Login link available on home page.
Read declaration and click on I Accept check box and click on Proceed button, after this screen will open like the below screen shot.
Above window contain two types of radio button for selection of Existing User and New User if you are a new user and want to start registration then click on New User and if you have already registered and want to take printout of application form then you can login from above screen.
For new registration you fill all information available on screen and after filling all information enter captcha code in text box according to displayed image and then click on submit button.
After clicking Submit button password will be sent on your registered mobile number, enter the password and click on “proceed” button
Then student dashboard with status will available on front of screen.
Now Click on User Activity Button on top of the Screen and select the option Fill Application Form, upload photograph, right hand index figure impression, signature and declaration form scan copy, accept the declaration, save and Update.

After filling application form again go to the user activity link and click on Pay Fees link
Pay fee according to available link on above screen click on Application Fee for UPCPMT-2015 link then you will be redirected to bank website Pay Application fee according to instruction available in this brochure.
After submission of application fee, candidate will receive a SMS for confirmation of fee deposit after 24 hour. Then he/she can proceed with printing his/her application form.
