Candidates seeking admission in Paramedical Courses in the state of UP have to fill their application form first to participate in the admission process. This year no entrance exam and interview. Admission will be based on the merit list, i.e., based on the 10+2 Mark-sheet.

How to Apply for UP State Medical Faculty Paramedical Admission 2022?

• Visit the website i.e forms.upsmfac.org.
• Click on Apply Now button or click on link given in the page to fill the form.
• Read the instructions carefully and check the “I have read the prospectus and the instructions Carefully and understood them” check-box. Click on Continue button.
• Fill all the personal details as per asked in the registration form. such as First Name, Middle Name, Last Name, Mobile Number, E-mail Address and click on Create new account button.
• An email shall be sent to your provided Email Address consisting of UPSMFAC ID and Password.
• Login with the UPSMFAC ID and Password sent to your registered email address.
• Upload the required documents in their prescribed format.
• Click on the payment option and pay the application fee via online mode.
• At last, submit the application form and take a printout for further use.
  Preferences
• Select your course preferences and fill up all the necessary information in the online form.
  • When Preference 1/2/3/4 (Diploma in X-Ray Technician) is selected from the dropdown, “Institute for XRay Technician” will be asked, where the respective institute name shall be shown. Minimum 1 institute or maximum all institute can be selected.
  • When “Diploma in Radiotherapy Technician” is chosen in “Preference 1/2/3/4”, then “Institute for Radiotherapy Technician” will be asked, where the respective institute name shall be shown. Minimum 1 institute or maximum all institute can be selected.
  • When “Diploma in Pharmacy” is chosen in “Preference 1/2/3/4”, then “Institute for Pharmacy” will be asked, where the respective institute name shall be shown. Minimum 1 institute or maximum all institute can be selected.
  • When “Diploma in Laboratory Technician” is chosen in “Preference 1/2/3/4”, then “Institute for Laboratory Technician” will be asked, where the respective institute name shall be shown. Minimum 1 institute or maximum all institute can be selected

Note :

• Under “Personal Information” section, if Category will be chosen as “Other” then “Please Specify” will be asked.
• Age should be 17 yrs or above
• In 12th Subject Details, Physics & Chemistry are mandatory and needs to filled up. Any one in between Maths or Biology is mandatory and needs to be filled up.
• For any other subjects of 12th other than Physics, Chemistry, Maths or Biology, use Subject 1 and Subject 2 options to fill in the details.
• Under “Address Section”, if Permanent Address is same as Present Address then click on “Yes”, else fill the “Permanent Address” section.
• Upload soft copies of passport size photo, signature, 10th & 12th mark sheets
• Click on I Agree of the Declaration
• Click on Submit to go to make an Online Payment. Don’t click on Back button while making payment.There are two online payment modes 1. Credit Card/Debit Card 2. Net Banking (Follow the instruction shown on the screen as per your choice)

• UP State Medical Faculty Paramedical Admission 2022 Selection Criteria for candidates will done in two stages.
• Only the candidates obtaining a positive (greater than zero) marks in Physics, Chemistry and Mathematics/Biology will be considered for all the subsequent stages.
• At different stages of the selection process ‘Application Rating’ (AR) marks of an applicant will be used.
• An applicant’s AR marks is the sum of the normalized marks ‘A’, and ‘B’ based on their percentage of marks obtained in the 10th standard and the 12thclass standard respectively.
• The marks in respect of candidates who have come from The Board of High School and Intermediate Education Uttar Pradesh will not be subjected to any Normalization. The marks obtained by such candidates in the subjects considered will be taken as such for ranking purposes.

For Marks AR A – for 10th Class

The marks received by candidate ion the exam will be considered as the main standard for satisfying the minimum eligibility criteria.

Other Boards

Consider a candidate from any other Board who is eligible and has applied to the exam.

Let us consider that average marks of applicants applying via that board is Yo where

Yo= Mean of Other Boards’ students’ marks, to be calculated as
Yo= {a1+a2+a3+………………+aN}/N

Where a1, a2, a3 are eligible candidates from other board who have applied.
Yo= {a1+a2+a3+………………+aN}/N

Normalizing Factor for the Other Board = Yo/Yup= k

The normalized marks of student at Other board= k*a1, k*a2, k*a3……….k*aN
which is AR Marks B of these Candidates.

FSC (Final Marks of Candidate)= {w1*AR Marks A} + {w2*AR Marks B

Where w1, w2 are percentage weightage assigned to AR Marks A and AR Marks B respectively.

Notes About the Normalization Norms:

• If any board/institution awards only letter grades without providing an equivalent percentage of marks on the grade-sheet, the candidate should obtain a certificate from the board/institution specifying the equivalent marks which should be used for filling the online application form.
• The original equivalence certificate needs to be submitted at the time of interview, if shortlisted for the same.
